First Visit

If this is your first visit to PolyHealth Physiotherapy Clinic, you may print and complete our New Patient Form in order to speed-up your initial appointment. Forms will also be available at our office.

What to Expect on Your Initial Visit?

  • Make yourself at home – Our friendly staff and doctors will do everything to make you feel right at home.

  • Consultation – Our doctors and practitioner will conduct a complete review of your medical history. The history is followed by a physical examination, including various tests, to allow the doctors to identify each client's physical barriers. Following the initial assessment, the doctor will have a discussion with you about the diagnosis, treatment options, risks, benefits, cost of treatment and individual goals. The initial visit usually takes about 1 hour.

  • Ask Questions – Our knowledgeable staff will be happy to answer any questions you may have regarding the recommended treatment, costs and procedures, or any other questions you may have.

  • Schedule Your Next Appointment – During the consultation, the doctor will customize a treatment plan for you which may include one or more therapy options. We will plan & schedule your appointments based on the treatment plan prescribed to you.

Payment & Coverage

Your treatments at PolyHelath Physiotherapy Clinic are fully or partially covered by private Extended Health Care plans, Auto Insurance or Workplace Safety and Insurance Board (WSIB). If you have medical health care coverage, your plan will cover some or all of the cost for Physiotherapy, Chiropractic Care, Massage Therapy, Acupuncture, Orthotics, and Medical Devices.

 

Please contact us to assist you in determining your coverage. If you are injured in a motor vehicle accident (MVA), your auto insurance will cover the cost of your treatments. We will assist you with your application forms and directly bill your auto insurance company with no cost to you. If you are injured at work, the cost of treatment is billed directly to the Workplace Safely and Insurance Board (WSIB) if your claim is approved.
